To confirm your phone number(s) for long distance calls have been switched to Startec please call (at no charge) 1-700-555-4141 to confirm your long distance carrier. You should hear a message from Startec.
Once your number(s) are activated, you only need to dial 1 or 011 before the long distance number.

I authorize Startec to make credit inquiries at any time in connection with the credit hereby applied for, and agree that the terms for using Startec's services & amendments thereof as Startec may stipulate from time to time shall apply to me upon acceptance of application. I understand Startec will forward a monthly invoice prior to the due date, reflecting charges incured &/or payments received during the latest billing cycle. I acknowledge that accounts are due within 15 days from invoice date and are subject to a late payment charge of 2.0% per month. Startec reserves the right to rerate calls to International cellular or audiotext numbers if deemed excessive by Startec.
By signing, I agree to subscribe the above telephone numbers to Startec's long distance service for Equal Access & authorize Startec to notify my local telephone company of my decision, so that every time I dial "1" or "011" plus the long distance number, the charges for the call will be billed by Startec. I have listed all phone numbers above that will be operated in this manner. I understand this authorization may be faxed, with the same force & effect as an original Equal Access authrization.
